NYS Changes Effective July 1, 2018
Section 136.3 regarding Health Examinations and Screenings will change. The new regulations for health examinations for attendance will be required for new entrants and in grades Pre-K or K, 1, 3, 5, 7, 9, and 11.
- The only form that may be used to document health examinations for students attending public school is the approved form titled; NYSED (New York State Education Department) Student Health Examination Form (See Forms)
- The new NYS School Health Examination Form is not required for physicals done prior to 7/1/18.
- To assist with the transition, during the 2018-2019 school year schools may accept any health examination form provided.
- In 2019-2020 ONLY the approved form will be accepted.
Hearing and Vision screenings will be done in 11th grade.
Vision screenings will now include near vision, as well as distance acuity.
Scoliosis screenings will be done for 9th grade boys only.
